Upload relevant information regarding your compassionate care request with Evolution CBD. An example of proof would include a recent doctor or hospital receipt.
Your application will be evaluated. You will receive an answer in 7 to 10 days.
If approved, you will receive confirmation that you are part of Evolution CBD’s Compassionate Care Group.
